Very cool little game! i jumped when i died the first time hahaha, really great vibes! i thought i wouldn't be scared because it wasnt realistic, but damn i was!
I think as it is its pretty good, but you might benefit from a few additional sounds coming from the npc's so they dont repeat the same noise constantly, when playing/being chased by something repeatedly, the constant sound loop can get a bit repetitive/annoying. just a few sounds to rotate between might make the npcs feel fresher and more alive.
on a semi related note to all of this, i do pixel art and have been wanting to work on some projects to expand my art portfolio, so if you ever want any assets for any games for free, hmu!
i almost forgot! a few items you could pick up and add to your inventory that help you through levels would make gameplay feel a lot more involved; even if its something like candy bars and energy drinks that temporarily boost your running stamina

You're absolutely right about the NPC sounds – I'm already adding variations to break the repetition.For the v0.02 content: I'm currently working on a new monster and an item system (including those stamina candy bars). I have some rough sketches. If you're interested in horror-themed assets, I could send you the concepts in private messages. Your pixel art could really bring these sketches to life.The inventory idea is brilliant. That's exactly where item sprites (and maybe some UI) will be needed.
